National Outreach Guidelines (Spanish) (2019). Resource Type: Publication. Description: Outreach is a critical function of health centers serving underserved populations. In 2000, Health Outreach Partners (HOP) developed its Farmworker Outreach Program Guidelines based on 30 years of experience providing direct outreach services to migrant and seasonal farmworkers. In 2012, HOP collaborated with four national partner organizations (the National Association of Community Health Centers, National Health Care for the Homeless Council, the Association of Asian Pacific Community Health Organizations and Migrant Health Promotion) and an Advisory Panel made up of representatives from community health centers and other support organizations across the country to update and expand on those guidelines, creating the current National Outreach Guidelines for Underserved Populations. Two Sides of the Same Coin: Addressing Social Determinants of Health and Enabling Services Data Collection (2019). Resource Type: Archived Webinar. Description: As health centers are held accountable for cost containment and high quality of care under value-based pay arrangements, it is increasingly important for providers to document the social determinants of health (SDoH) barriers of their patients and what services they are providing (both clinical and non-clinical) in order to properly care for those complex patients. Some of these services are called enabling services (ES), defined as non-clinical services that aim to increase access to care and improve health outcomes. Standardized data collection on SDoH and ES are two sides of the same coin and provide the foundation for health centers to succeed in a value-based pay environment.
